  1. What documents are required to sign a declaration for a child and for an adult?

To sign a declaration with a family doctor (according to the rules of the National Health Service of Ukraine), certain documents are required. The requirements differ for adults and children. For adults: ◾ Ukrainian passport or another identity document (ID card, passport) ◾ Taxpayer Identification Number (if available) ◾ Mobile phone – for SMS confirmation For children: ◾ Child’s birth certificate ◾ Passport of one parent or guardian ◾ Child’s Taxpayer Identification Number ◾ Document confirming guardianship or custody – if the declaration is signed by someone other than the biological parent ◾ Parent’s/guardian’s mobile phone – for SMS confirmation

2. What should I do if I want to change my family doctor?

To change your family doctor, you need to sign a new declaration with another doctor. The previous declaration is automatically canceled at the moment the new one is signed – no additional steps are required. Steps to follow: Choose a new doctor. You can use the Helsi.me service or contact the medical facility directly. Make sure the doctor has available slots for new patients. Prepare the required documents: ◾ Passport (ID card or booklet) ◾ Taxpayer Identification Number ◾ Mobile phone for verification For children – also the birth certificate, Taxpayer Identification Number, and one parent’s passport. Sign the declaration with the new doctor at the medical facility.

3. My doctor is on vacation / ill / on maternity leave – what should I do?

If your family doctor is unavailable for any reason, you can contact the doctor who is covering for them. To find out which doctor it is, you can call the reception, check this information on the website, or on our social media channels.

4. Can a family doctor provide a consultation at home?

A family doctor can make a home visit in two cases: ◾ when the service is officially paid for according to the current fee schedule for paid medical services ◾ if the family doctor decides that a home consultation is necessary based on the patient’s condition

5. Can laboratory tests or an ECG be performed at my home?

Collection of laboratory tests or performing an electrocardiogram (ECG) at home is not covered by the Program of Medical Guarantees. However, these services can be provided by our facility on a paid basis according to the current fee schedule for paid medical services.

6. Who is responsible for issuing a Medical Death Certificate on weekends?

A Medical Death Certificate can be issued by the on-duty doctor on Saturdays. Please inform your family doctor about the death of a relative during working days.

7. On which days does the Medical Advisory Commission (MAC) operate?

The Medical Advisory Commission operates on Tuesday, Wednesday, and Thursday from 11:00 to 13:00.

8. Can a family doctor issue me referrals for consultations and treatment?

A family doctor can issue referrals: ◾ to specialists (cardiologist, endocrinologist, dermatologist, neurologist, etc.) ◾ for laboratory tests (complete blood count, urine test, biochemistry, etc.) within the scope of their consultation ◾ for instrumental examinations (ultrasound, X-ray, ECG, mammography) within the scope of their consultation ◾ for hospitalization or additional inpatient examinations Please note that once a family doctor refers a patient to a specialist, any further referrals needed to establish a diagnosis or perform tests are issued exclusively by that specialist. 9. How to make an appointment?

You can make an appointment with your family doctor through your personal account in the HELSI app or by calling the reception.

10. What is the «Affordable Medicines» program?

In April 2017, the Government launched the “Affordable Medicines” reimbursement program to reduce the financial burden on patients and improve access to medications. Thanks to this program, people with chronic conditions can receive the necessary medicines free of charge or with a partial co-payment. You can get an e-prescription for medications included in the “Affordable Medicines” list from your family doctor.

11. What is the Medical Guarantees Program (MGP)?

The Medical Guarantees Program (MGP) is Ukraine’s state system that ensures citizens receive free medical services within the budget allocated by the National Health Service of Ukraine (NHSU). The MGP covers a list of medical services and medications that: ◾ are funded by the state ◾ are free for the patient ◾ are provided via referral or, in emergencies, without one What does the MGP provide? ◾ Free medical care under the program’s conditions ◾ Equal access to treatment, regardless of location, age, or social status ◾ Transparent funding: hospitals receive payments from the NHSU for the services actually provided What is included in the Medical Guarantees Program? ◾ Family doctor services (through a signed declaration) ◾ Emergency medical care ◾ Outpatient care (consultations, lab tests, ultrasound, X-rays, etc.) ◾ Inpatient treatment ◾ Childbirth ◾ Surgical interventions ◾ Treatment of stroke, heart attack, and cancer ◾ The “Affordable Medicines” program ◾ Mental health services Important: To use MGP services, you need a declaration with a doctor or an electronic referral.

12. Where and when can I check the results of my analysis?

Usually, test results are ready on the same day (in some cases – the next day, depending on the laboratory’s workload). You can view the results of any tests or examinations in your personal account in the HELSI app, under the «Medical Record» section in the “Medical Documents” tab, or by contacting your family doctor.

13. Who is responsible for issuing an electronic sick leave certificate?

An electronic sick leave certificate (MVTN – medical certificate of temporary incapacity) is issued by the doctor managing the episode of illness that caused the temporary incapacity.

14. What information must you provide to the operator when calling an ambulance?

When calling an ambulance, it is very important to provide clear and accurate information so the operator can assess the situation and dispatch a team as quickly as possible. Essential information to provide when calling an ambulance: ◾ exact address of the incident ◾ town or city, street, building number, entrance, floor, intercom code (if needed) ◾ what happened: complaints or symptoms, e.g., loss of consciousness, seizures, chest pain, high fever, injury, etc. ◾ number of people affected: if multiple people are involved, specify how many need assistance ◾ patient’s approximate age and gender, e.g., “male, about 60 years old,” “child, 5 years old” ◾ whether the patient is conscious, breathing, has a pulse, e.g., “unresponsive, not breathing,” “conscious but very weak pulse” ◾ your contact information: name and phone number for follow-up ◾ any additional details, if relevant, e.g., pets at home, locked doors, need for police or rescue services, important existing medical conditions The most important thing is not to panic. Answer the dispatcher’s questions clearly, even if there seem to be many – it saves time and can save lives.
